PARTICIPANT WAIVER, RELEASE OF LIABILITY & CONSENT

In consideration of being permitted to participate in the Santa 1K Run & Walk (“Event”), I acknowledge and agree to the following:

ASSUMPTION OF RISK

I understand that participation in a running or walking event involves inherent risks, including but not limited to falls, collisions with other participants or spectators, uneven or slippery surfaces, road and sidewalk conditions, weather conditions, traffic, physical exertion, illness, injury, and other risks associated with participating in an outdoor athletic event.

I voluntarily choose to participate and knowingly assume the risks associated with my participation.

HEALTH AND FITNESS

I certify that I am physically capable of participating in this Event and have not been advised by a medical professional that I should refrain from participating.

I understand that I am responsible for determining whether I am sufficiently healthy and physically prepared to participate.

RELEASE AND WAIVER OF LIABILITY

To the fullest extent permitted by applicable law, I release and hold harmless Johnstown Lifestyle, its directors, officers, employees, volunteers, representatives, agents, sponsors, event partners, and affiliated organizations, as well as the City of Johnstown, Ohio, and its officials, employees, volunteers, representatives, and agents, from claims arising from or related to my voluntary participation in the Event, except to the extent such claims cannot legally be waived or released.

I understand that this waiver is intended to apply to ordinary risks associated with participating in the Event and is not intended to waive any rights or claims that cannot legally be waived under Ohio law.

EMERGENCY MEDICAL CARE

If I become injured or ill during the Event and I am unable to provide consent, I authorize Event personnel to seek reasonable emergency medical assistance on my behalf.

I understand that Johnstown Lifestyle does not assume responsibility for medical expenses incurred as a result of my participation.

EVENT CONDITIONS, WEATHER AND CANCELLATION

I understand that the Event is scheduled to take place outdoors and may be affected by weather, road conditions, emergencies, governmental directives, or other circumstances beyond the control of Johnstown Lifestyle.

For the safety of participants, organizers may modify, postpone, shorten, or cancel the Event when circumstances reasonably require it.

PHOTOGRAPHY AND VIDEO

I understand that photographs and video may be taken during the Event. I grant Johnstown Lifestyle permission to use photographs or video in which I appear for legitimate nonprofit, promotional, informational, social media, website, and community-outreach purposes without compensation.

PARTICIPANTS UNDER AGE 18

If the participant is under 18 years of age, I certify that I am the participant's parent or legal guardian or otherwise have legal authority to provide the permissions and acknowledgments contained in this registration.

I consent to the minor's participation in the Event, acknowledge the risks associated with the minor's participation, and agree to the provisions of this waiver on my own behalf.

ACKNOWLEDGMENT

I have had the opportunity to read this Participant Waiver, Release of Liability & Consent before registering.

I understand its terms and voluntarily agree to them.
