BACK TO SCHOOL CHILDHOOD IMMUNIZATIONS

Provided by the Licking County Health Department Mobile Unit
Call the LCHD at 740-349-6535 and follow the prompts to schedule an immunization appointment (option 5). When scheduling with one of their staff, ask for scheduling at the Johnstown Farmers Market.
The LCHD staff will ask for insurance information for your child and any vaccination records you have at the time of the call.
Appointment scheduling will be open until 12pm on Tuesday, August 4.
Parents or guardians should arrive at the LCHD Mobile Unit at the farmers market at their scheduled time.
